- Goal #1: To increase the number of children
between the ages of 0 and 5 who meet age appropriate developmental milestones.
Objective #1: While enrolled in the program,
90% of target children will meet age appropriate developmental milestones, as measured by the Ages and Stages, Denver, or other standardized developmental screening tools.
Objective #2: While enrolled in the program, 100% of target children with suspected delay, as identified by standardized developmental screening tools,
will be given referrals to appropriate services, and 95% of parents will follow through on these referrals and recommendations.
- Goal #2: To increase access to and utilization of health care services by families.
Objective #1: While the family is enrolled in the program, 95% of families will have a primary care provider.
Objective #2: While the family is enrolled in the program, 95% of target children will receive on-time, age appropriate immunizations required by the American Academy of Pediatrics (AAP).
Objective #3: While the family is enrolled in the program, 95% of target children will receive AAP recommended well-child visits.
Objective #4: While enrolled in the program, 95% of women who become pregnant will begin prenatal care in the first trimester, and will receive the American College of Obstetricians and Gynecologists' recommended number of prenatal visits.
- Goal #3: To increase positive parenting and decrease the number of category 1 or 2 substantiated cases of child abuse and neglect.
Objective #1: While enrolled in the program, 95% of families will not have a category 1 or 2 disposition (substantiated reports) of child abuse or neglect.
Objective #2: 95% of parents responding to satisfaction surveys will report that their parenting improved as a result of participating in the program.
